Support for Diabetes Patients Offered at Northwest Health in La Porte

Northwest Health – La Porte offers diabetes support and education programs to individuals throughout the region. 

Diabetes Self-Management Class

This four-week class is a comprehensive program helping empower individuals with diabetes to control their own health. The next class will start on June 3 and meet each Monday in June from 9 – 11:30 a.m. at Northwest Health – La Porte. The hospital is located at 1331 State Street, La Porte, Indiana. A physician's order and pre-registration is required.

Diabetes Support Group

The Diabetes Support Group provides those with diabetes a place to share ideas, experiences, and concerns related to diabetes. The group meets on the fourth Monday of the month from 11 a.m. – Noon in the Lily Lake Conference Room on the fourth floor of Northwest Health – La Porte hospital. No registration is required.
