Healthcare Foundation of La Porte Announces First Health and Wellness Grant Application Cycle to Open June 16, 2017

healthcare-foundation-laporte-houseThe Healthcare Foundation of La Porte (HFL) is excited to announce that the 2017 Grant Application Cycle will open on June 16, 2017. All grant applications will open on this date, and HFL has budgeted $3.5 million for local organizations whose proposed programs and services empower residents to live healthy and well in and around La Porte.

To prepare for this first year of grants, HFL will be hosting two announcement meetings to share information about the grant cycle timeline, the types of grants being offered and HFL’s three Strategic Focus Areas. These sessions are an opportunity to share and discuss this information with the organizations who plan to apply for an HFL grant so that representatives of these organizations can brainstorm collectively and ask questions about the grant process.

“This first year of HFL awarding grants is incredibly exciting but is also new,” said Maria Fruth, President & CEO of HFL. “We want to use this time as an opportunity to continue building relationships with the organizations who plan to apply for grants to encourage collaboration throughout the grant cycle.”

The meetings will take place on Thursday, June 15 from 4–6pm or Friday, June 16 from 8–10am at the La Porte County Public Library Main Branch (904 Indiana Ave). Based on limited space, HFL requests that no more than two individuals per organization attend. In addition to hearing from Maria Fruth, attendees will have the opportunity to listen to a presentation from Justin Rivas, MPH, MIPA – Community Coach, to learn about County Health Rankings. The County Health Rankings and Roadmaps Program is a partnership of University of Wisconsin Population Health and The Robert Wood Johnson Foundation. These rankings provide a revealing snapshot of how health is influenced by where we learn, work and play. Mr. Rivas will be present at these sessions to provide information about how County Health Rankings are assigned, what these rankings mean for our community and how to use these rankings as a starting point for change in our community.

This site provides County Health Rankings data, a Community Scan compiled by The Northern Trust Company, a La Porte Health Needs Assessment done by local hospitals and organizations, and information about local health and wellness resources. HFL has been working to compile and promote information about organizations that attend the health and wellness needs of the community. “Ten 2030 is a resource for the community. Our hope is that residents of La Porte will use Ten 2030 to find health and wellness programs and services that meet their needs and local organizations will use the data to determine what needs in the community still have to be met, best practices for creating programs to meet those needs, and funding sources for those programs,” said Fruth.
