IU Health LaPorte Hospital Sports Medicine, in conjunction with the LaPorte High School Athletic Department, will be offering high school sports physicals for the 2014-2015 school year on Thursday, June 5th from 5:00 to 8:00 p.m. in the High School gymnasium. All high school students who wish to participate in athletics during the ‘14-‘15 season are encouraged to take advantage of this opportunity.
All athletes must have a yearly physical before participating in athletics. Any high school athlete planning to participate in summer conditioning must have a physical on file with the LPHS athletic office on or before June 5th, 2014. Athletes are asked to pick up the physical form at the high school athletic office and have all necessary signatures and forms completed prior to their physical.
Because of limited space and giving highest quality athletic screenings, we will have two available dates this summer for all other athletes in the community to get sports physicals. These sports physicals will be held at the IU Sports Medicine Center, 600 Legacy Plaza E, La Porte, IN 46350 on Thursday, July 17th and Thursday, August 14th from 8am to 3pm. Any athlete is welcome to walk-in during that time to get their sports physical.
